If the permit or waiver is damaged and needs to be replaced, the Central Permit Office will allow reissue without completing a new application if the physical tag or card is brought to the office for the exchange. If your permit or waiver has been confiscated for misuse, we will not replace it. Please use your permit or waiver as directed to prevent confiscation.

Applications must be submitted in person to the Central Permit Office. Do not go to the DMV.
Come to the Central Permit Office, located on the first floor of One Park Place, 300 South State Street. The main entrance is located on the south side of the building, on East Onondaga Street. Parking is available in the One Park Place parking lot, located at 343 East Onondaga Street. All visitors are asked to go through a security check.
